A Home Medicines Review, or HMR, is a free, government-funded service for eligible Medicare and DVA patients. It provides a comprehensive review of your prescription medicines, non-prescription products, vitamins and supplements. A credentialed pharmacist visits you at home, discusses how you manage your medicines and provides a report to your GP or referring medical practitioner.

An HMR may help if you take several medicines, have recently returned home from hospital, experience possible side effects, find your medicine routine difficult to manage or have questions about your treatment. Your GP or eligible medical practitioner will assess whether an HMR is clinically appropriate for you.

Yes. The HMR service is government funded and provided at no cost to eligible Medicare and DVA patients. You may still need to pay for your appointment with the referring medical practitioner if it is not bulk billed.

We will discuss all the medicines and health products you use, how you take them and any questions or concerns you may have. This includes prescription medicines, pharmacy or supermarket medicines, vitamins, supplements, inhalers, creams and medicines used only when needed.

A home visit generally takes approximately 60–75 minutes, depending on the number and complexity of your medicines and health needs.

Please gather all your medicines, vitamins and supplements before the visit. Include inhalers, creams, patches, eye drops, medicines used only when needed, dose administration aids and any recent hospital discharge information. A family member or carer is welcome to be present.

Gwen will prepare a report for your GP or referring medical practitioner outlining any medicine-related concerns and recommendations. Your doctor will discuss the findings with you and agree on any appropriate changes or next steps. Please do not stop or change prescribed medicines unless advised by your prescriber.

Home Medicines Reviews are available across Warragul, Drouin, Yarragon, Moe, Morwell, Traralgon and selected surrounding Gippsland communities. Please contact Gwen if you are unsure whether your location is covered.

Mobile vaccination clinics may be available for workplaces, community groups, school and childcare staff, aged-care facilities and other group bookings. Contact Gwen with your location, approximate participant numbers and preferred timeframe to discuss availability.

This is a privately funded phone or video consultation with a registered pharmacist. It provides dedicated time to discuss your medicines, supplements, side effects or health questions. It is separate from the government-funded Home Medicines Review Program and does not require a referral.

Telehealth consultations are private services and are not funded through Medicare. The consultation type and fee will be confirmed with you before booking.

A pharmacist may provide an absence-from-work or carer certificate for a short period where clinically appropriate. You will need to complete an assessment, and a certificate cannot be guaranteed.

No. Certificates cannot be backdated and are only issued following an assessment of your current circumstances.
