Home Medicines Reviews are government funded. A referral from your GP or eligible medical practitioner is required.

How it Works

How a Home Medicine Review Works

Speak with your GP

Book an appointment with your GP and ask whether a Home Medicines Review may be suitable for you.

Your GP sends a referral

If appropriate, your GP sends the HMR referral to: gwen@yourhealthally.com.au

Contact will be made

Once your referral is received, Gwen will contact you to discuss the service and arrange a suitable time for your home visit.

Your Home Medicines Review

The consultation takes place in the comfort of your home and generally lasts 60–75 minutes, depending on the complexity of your medicines and health needs.

Follow up with your GP

Your GP will discuss the recommendations with you and work with you to agree on any appropriate changes or next steps.

A report is sent to your GP

Following the review, Gwen prepares a detailed report outlining any medicine-related concerns and recommendations for your GP to consider.
