The Home Medicines Review Program helps people better understand and manage their medicines. A credentialed pharmacist conducts a comprehensive review in the person’s home to support the safe and appropriate use of medicines and reduce the risk of medicine-related harm. The pharmacist works together with the patient, their GP or eligible medical practitioner, and other members of their healthcare team to support better medicine management.
What is a Home Medicines Review?
A Home Medicines Review, also known as an HMR, is a comprehensive review of your prescription medicines, non-prescription products, vitamins and supplements.
The consultation takes place in your home, where we can discuss how you actually use and manage your medicines in everyday life.
Following the review, I prepare a report for your GP outlining any medicine-related concerns and practical recommendations for your GP to consider. You and your GP then discuss the findings and agree on any appropriate changes or next steps.
An HMR does not replace the care provided by your GP. It brings you, your doctor and your pharmacist together to support the safe and effective use of your medicines.
