Your local pharmacist and health ally
Hi, I’m Gwen Yap, a Warragul-based credentialed pharmacist, pharmacist immuniser and founder of Your Health Ally.
With more than 10 years of experience in community pharmacy, including pharmacy management, I have supported people through many stages of their health journey—from starting new medicines and managing chronic conditions to navigating treatment changes after a hospital stay.
What I have learnt is that people often need more than a quick answer. They need time to ask questions, understand their options and feel heard.
That is why I created Your Health Ally.
Why Your Health Ally exists
Medicines and health information can quickly become confusing, particularly when several conditions, medicines or healthcare professionals are involved.
Your Health Ally was created to provide thoughtful, pharmacist-led support focused on clarity, medication safety and what matters most to each person.
My goal is to help people:

I provide Home Medicines Reviews across Warragul and selected areas of Gippsland.
During a Home Medicines Review, I take the time to understand how you use your medicines in everyday life, listen to your concerns and explore practical ways to make your medicines safer and easier to manage.
Following the review, I provide your GP with a clear, clinically relevant report to support collaborative decision-making and ongoing care.

My approach
I believe good healthcare begins with listening.
My approach is warm, practical and person-centred. Rather than looking at medicines in isolation, I consider the whole person—including their health goals, daily routine, concerns and individual circumstances.
I value working collaboratively with patients, families, GPs, community pharmacies and other healthcare professionals to support the safe and effective use of medicines.
Supporting my community
As a local pharmacist and mother of two, I understand how difficult it can be to navigate healthcare while managing everyday life.
I am passionate about improving health in my community through medication management, health education and accessible pharmacist-led care.
Alongside Home Medicines Reviews, Your Health Ally also provides pharmacist telehealth consultations and mobile vaccination clinics across selected areas.
