How to Use Your Medication Summary Chart

  1. Fill in your details Write your name, address, and date of birth at the top.

  2. List your medications Include prescriptions, over-the-counter medicines, and supplements. Add instructions and notes such as tablet colour, size, or special requirements.

  3. Track daily doses The left-hand column lists the days (Monday–Sunday).Each column (Morning, Noon, Evening, Bedtime) shows what to take. Example: Morning – 1x Aspirin, 2x Paracetamol

  4. Use with or without a dosette or webster pack

    • With a dosette or webster pack: Pack medications according to the chart (Monday–Sunday, 4 times daily).

    • Without a dosette or webster pack: Print the chart and follow it each day.

  5. Keep it accessible Place the chart on your fridge, near your medication area, or by your bedside.

  6. Review and update regularly Bring the chart to your pharmacist or doctor for review, especially if your medicines change.
